COVID Vaccine Proof Letters – My patient is asking for evidence of their COVID vaccination for travel abroad what should I do?

GPs cannot provide letters showing peoples COVID-19 vaccination status.

You can access your COVID-19 vaccination status through the free NHS App from 17 May. You can access the app through mobile devices such as a smartphone or by tablet. Proof of your COVID-19 vaccination status will be shown within the NHS App. We recommend that you register with the app before booking international travel.

If you do not have access to a smartphone and know that the country you are travelling to requires COVID-19 vaccination status, you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine. We expect the letter to take up to 5 days to reach you.

My patient has requested an early second dose of the COVID vaccine as they are due to travel abroad for a holiday, what should I do?

We cannot arrange early COVID vaccination for people travelling abroad.

People who are travelling abroad who are not fully vaccinated should follow the government advice:

People should continue to follow the entry requirements of the country they are travelling to, such as proof of a negative COVID-19 test on arrival. You should carefully research the requirements of your destination country before travelling.

My patient is not currently in an eligible cohort for COVID vaccination and is requesting a COVID vaccine so that they can travel abroad what should I do?

COVID-19 vaccine clinics

If you or your family member or the person you care for have an appointment for the COVID-19 vaccine, the booking team will discuss over the phone, the information you need, when they book your appointment.

  • Consent forms, patient letters, patient information leaflets and advice can be found on the GOV UK website.

The COVID-19 vaccine is NOT being done at Elliott Street Surgery.

The vaccines are being delivered to a local central location, where there are facilities for the cold storage of the COVID-19 vaccine.

  • Where to go for your COVID-19 vaccine appointment 

The vaccination centre is at:  Leigh Leisure Centre, Leigh Campus, Sale Way, Leigh Sports Village, Leigh, WN7 4JY  (directions as below)

There will be staff at the entrance greeting people.

Directions by car:

  • Drive down Sale Way, the Leigh Sports Village stadium will be on your left and Morrison’s on your right.
  • Follow the road to the very end and Car Park 1 is on the left.
  • Look out for the NHS vaccination clinic signs to help you.

After the COVID-19 vaccine, there may be a 15 minute observation period depending on which of the COVID-19 vaccines you have been given, so please plan your return journey accordingly.

Please do not turn up at Leigh Leisure Centre without an appointment.
